Chronic Disease Management in Singapore

If you have been diagnosed with diabetes, hypertension, high cholesterol, or another long-term condition, you deserve more than a repeat prescription. You deserve a structured, consistent chronic disease management plan that helps you stay well for the long term.

At T&T Medical Group, we’re equipped to provide exactly that.

What Is Chronic Disease Management?

Chronic disease management is a structured approach to caring for conditions that require ongoing medical attention rather than a single course of treatment.

Rather than addressing symptoms only when they flare up, this approach focuses on regular monitoring, medication review, and lifestyle support to reduce complications and maintain your quality of life.

In Singapore, conditions such as diabetes mellitus, hypertension, and hyperlipidaemia are among the most prevalent long-term health concerns, with around 1 in 3 residents affected by hypertension or high cholesterol alone.

Left unmanaged, these conditions may increase the risk of serious complications, including cardiovascular disease, kidney damage, and nerve injury.

Structured management aims to slow disease progression, reduce that risk, and help you feel more in control of your health. For many patients, having a consistent plan in place makes it easier to stay on top of their condition between appointments and to understand what changes to look out for over time.
